		  <strong>7.</strong>I agree to the following:-<br><br>
		  <strong>a.</strong> I hereby agree to pay minimum subscription of Rs 550 /- (i.e. Rs 50/- for non-refundable admission fee and Rs 500/- for five shares of Rs 100/- each) and also agree for deduction of additional amount towards share capital proportionate to milk supply at an appropriate time as decided by the Company.<br><br>
            <strong>b.</strong>	For the same, I have deposited Rs 150/- in Company’s bank account and attaching the corresponding bank slip/DD along with the form. I agree to get the remaining amount of Rs 400/- deducted from my milk bill payments within 3 months of starting of the MPP where I can pour milk or my code activation, whichever is later.<br><br>
            <strong>c.</strong>	I understand that Company can cancel my membership if I am found defaulting on point no. 7a and 7b. I also understand that the admission fee of Rs 50/- is non-refundable. <br><br>
            <strong>d.</strong>	As per point no. 4.3 in Articles of Association of the Company as resolved by the Board, I agree to subscribe additional shares matching the annual quantity of milk supplied (calculated at the rate of Rs.1/- or any other amount specified by the Board from time to time for every litre of annual quantity of milk supplied). In this regard, I authorise the Company to deduct the above mentioned amount from my milk bill payments towards additional share capital in order to match my milk supply during the year and issue share certificate of additional amount accordingly after approval by the Board.<br><br>
            <strong>8.</strong>	I am given to understand that after being enrolled as a member, I would need to meet all the conditions for continuation of membership. Based on the quantity supplied, I shall need to subscribe additional share capital and would then be made a ‘Class A’ or ‘Class B’ or ‘Class C’ member. In addition to the quantity supplied, I undertake to abide by other conditions laid down by the Company for being a ‘Class A’ or ‘Class B’ or ‘Class C’ member.<br><br>
            <strong>9.</strong>To retain my membership, I hereby undertake to supply all the surplus milk available in the household to the Company, maintain the required milk supply ratio and adhere to milk quality standards as stipulated by the Company from time to time. <br><br>
            <strong>10.</strong>I agree to the following;<br><br>
            <strong>a.</strong>	The Company shall, as decided from time to time, pay dividend on the share capital subscribed by me.<br><br>
            <strong>b.</strong>	The Company may de-activate my member code for the remaining financial year if milk supply in my code exceeds my matching share capital contribution as a result of which I may not be able to pour milk.<br><br>
            <strong>c.</strong>	The total milk supplied to the Company during the winter months namely November to February shall not exceed such times as may be decided by the Company from time to time compared to milk supplied during the summer months namely April to July during each financial year.<br><br>
            <strong>11.</strong>I am given to understand that -<br> 
            a.	The Company will provide membership related benefits only to those members who fulfil the membership continuation criteria. <br><br>
            b.	The company shall cancel the membership if the member indulges in any act which is detrimental to the functioning of the Company or which has damaged or likely to damage the interest and reputation of the Company.<br> <br>
            c.	My membership shall not be continued with the Company if I am not fulfilling the conditions for continuation of membership as given in annexure to this application.<br><br>
            <strong>12.</strong>I undertake to abide by all the conditions as per Companies Act 1956 Part IX-A (Companies Act 2013 and its latest provisions as may be applicable), Memorandum of Association and Articles of Association, Board and AGM resolutions as decided from time to time.<br><br> 
            <strong>13.</strong>I understand that in case my membership application is not accepted by the Company or if it is cancelled at a later point of time, then my share capital amount would be reimbursed to me through cheque/DD/bank transfer only. Therefore, it is my responsibility to share correct bank details with the Company.<br><br> 
            <strong>14.</strong>In case any dispute arises, the courts of ______ will be the jurisdiction to resolve the matter. The dispute shall be settled by arbitration in accordance with the Arbitration and Conciliation Act, 1996. The sole arbitrator will be appointed by CE of ________MPC.<br><br>
            <strong>15.</strong>I have read/got it read and understood the conditions mentioned as point 7 to 14 and agreeing to abide by it. I am also signing a “Declaration” as a token of acceptance.<br><br> 
            <strong>16.</strong>I am here with declaring that myself or any one of the family member are not enrolled as member in ________ Milk Producer company limited and for the first time applying for enrolment. <br><br>
            <strong>17.</strong>	I undertake that the details given above are true and I understand that the Company has the right to accept or reject the application. I have enclosed copy Aadhaar Card/ Bank pass book / PAN Card / Voter ID card /Driving License as a proof.<br><br>

		  <strong class="text-center fs-17"><h3>Declaration by the Member Applicant</h3></strong>
		  <br><div class="col-md-12">
			<strong>If my application is accepted and membership is provided in the Company, I hereby declare that I will fulfill all the terms and conditions as stipulated by the Company from time to time and abide by the same. I understand that in case I fail to meet the criteria as mentioned below, my membership will be cancelled from the Company.</strong><br><br> 
            <strong>1.</strong>	The member is engaged in production of milk from milch animals owned by her and responsible for the care and management of cow(s) and buffalo(es).<br><br>  
            <strong>2.</strong>	The member has supplied milk for at least 200 days and poured a minimum of 500 litres of milk during the previous financial year. <br><br>
            <strong>3.</strong>The total milk supplied to the Company during the flush months namely ________ to __________ shall not exceed such times as decided by the company during the lean months namely  ________  to __________  in a financial year. <br><br>
            <strong>4.</strong>	The member has subscribed for shares matching the annual quantity of milk supplied. <br><br>
            <strong>5.</strong>	The milk supplied by the member has met all the quality standards as stipulated by the Company from time to time. <br><br>
            <strong>6.</strong>	The Membership may also be cancelled if …   <br><br>
            a.	More milk quantity poured in the member code in comparison to the actual milk production/surplus in the household as declared by the members <br><br>
            b.	Fake membership including more than one members in the same household (including the  Sahayak family)<br><br>
            c.	Member code is used to pour milk of somebody else <br><br>
            d.	If membership is allotted in  Sahayak’s name<br><br>
            In addition, <br><br>
            <strong>7.</strong>	Membership shall continue without being disqualified so long as, <br><br>
            a.	The Member does not have/acquire any business interest, which is in conflict with the business of the Company.<br><br>
            b.	The member is not indulging / has not indulged in an act, which is detrimental to the functioning of the Company or which has damaged or likely to damage the interest and reputation of the company. <br><br>
           c.	The member has not wilfully deceived the Milk Producer Company.  <br><br>
           d.	The Member has not provided any incorrect information to the Company. The company can verify the information provided by the member on suo moto basis or on receiving complaint from a bona fide member.<br><br>
            e.	The Member has not been found defaulted in supplying the entire surplus milk of her household to the Company. <br><br>
